At Little Cross Family Clinic, we help parents and guardians by assisting with the submission and verification of immunisation records through the National Immunisation Registry (NIR)

Why Childhood Immunisation Matters

In Singapore, it is a legal requirement under the Ministry of Health (MOH) that all foreign-born children must be vaccinated against diphtheria and measles before applying for passes such as:

Student Pass
Long-Term Visit Pass (LTVP)
Dependant’s Pass (DP)

These vaccinations must be verified by the Health Promotion Board (HPB) before your child’s pass application can proceed. Without proper verification, your child’s application may be delayed or rejected.

Our Services for Childhood Immunisation Verification

We provide two main services:

✅ Filling of Immunisation Form – $80

We help complete the hardcopy immunisation form accurately and get it endorsed by our doctor.

✅ Filling + Submission of Form – $155

We handle everything for you, including online submission to the Health Promotion Board (HPB).

Document arrangement – $15
Urgent (same-day submission before 4pm) – $20 surcharge

Documents Required for Application

Please prepare softcopies only (no hardcopies accepted)

For the Child:

Valid travel document (at least 6 months validity)
Birth certificate
Immunisation registration form (certified by a doctor)
Immunisation records from country of origin

For the Parent/Guardian:

Passport (minimum 6 months validity)
Singapore ID with FIN (if any)

If documents are not in English:

Provide original copy and official English translation

Translation must be done by an embassy, notary public, or certified translation service

Processing Timeline

Processing takes around 20 working days (excluding weekends and public holidays).
Once approved, HPB will email you the Verification of Vaccination Requirements (VR) document, which you can download and use for your child’s immigration pass application.

Application fee payable to HPB is $35 (non-refundable) via VISA/MasterCard.

We operate on a WALK-IN BASIS.
To avoid disappointment, we encourage patients to come earlier, as registration may close earlier when capacity is reached
